docs: add screenshot gallery to README

Add 4 app screenshots in a 2x2 gallery:
- Home screen with saved connections
- Data grid with FK preview
- Interactive ER diagram / schema visualizer
- PostgreSQL enum detail view

Images are stored under screenshots/ and embedded above
the Features section.
